# OMBI®

The OM1 Medical Burden Index (OMBI®) measures the combined medical burden of an individual or a patient population. Using the PhenOM® foundation model and OM1 Real-World Data Cloud of over 400 million patients, OMBI® creates a global risk score for overall episode cost.

## A global risk score for overall episode cost

OMBI® provides a single composite 0–100 score estimating a patient's overall risk of an episode exceeding its target price. Built from procedure- specific predictions, OMBI® applies uniformly across all 5 TEAM categories and CJR-X to give care teams one number to prioritize attention.

## CMS TEAM

*5 mandatory episode categories, live since January 1, 2026.*

- Lower Extremity Joint Replacement (LEJR): 30-day episode
- Surgical Hip/Femur Fracture Treatment (SHFFT): 30-day episode
- Spinal Fusion: 30-day episode
- Coronary Artery Bypass Graft (CABG): 30-day episode
- Major Bowel Procedure: 30-day episode

## CMS CJR-X

*Nationwide mandatory LEJR model. Begins January 1, 2028.*

- Hip, Knee & Ankle Replacement (Inpatient + Outpatient): 90-day episode
- Predictions include: 90-day all-cause readmission risk, 7-day unplanned visit risk (outpatient), discharge disposition (home vs. SNF/IRF),  periprosthetic joint infection risk, VTE risk (DVT/PE), post-op complication composite risk, knee and hip preoperative PRO functional outcome improvement prediction
